Health Examination Forms 

Students who will be entering kindergarten, sixth grade, and are new to the district must provide verification of a complete health examination and administration of the following required immunizations prior to the first day of school. A physician (licensed to practice medicine in all of its branches) must review and sign all required report forms. The medical history section of the form may be completed and signed by the parent or legal guardian of the student. The requirements regarding immunization for students in the District shall comply with the standards adopted by the Illinois Department of Public Health. Every student shall present proof of immunization for preventable communicable diseases.

The school nurse shall appraise parents of their obligation in these areas and shall furnish the necessary forms and keep records of compliance. If the student’s physical condition is such that any one or more of the immunizing agents should not be administered, the examining physician shall so state on the examination form.

Any child not in compliance with the state mandated health requirements by October 15th will be excluded from school that day and until proof of compliance is presented to the school. All new students who are first-time registrants, entering after October 15th, shall have 30 days following registration to comply with the health examination and immunization regulations.

Sports Physicals 
A new sports physical must be turned in annually. Students may try out for interscholastic sports teams, but may not practice or participate until a current sports physical is on file. Sports physical forms are only valid for playing sports, and will not be accepted as a mandated health examination.

Dental and Vision Examinations
Effective July 1, 2005 all children in kindergarten, second, and sixth grades, and students new to  Illinois schools will be required to have an oral health exam. Each child shall present proof of examination by a dentist prior to May 15th of the school year, or proof that a dental examination will take place within 60 days after May 15th.  

Effective January 1, 2008, all children enrolling in kindergarten will be required to have an eye examination. Each child shall present proof of examination by an optometrist or physician who provides complete eye examination by October 15th of the year the child is first enrolled. The examination must be completed within one year prior to the child beginning school.
